The
Grace Hospice Team consists of members who focus not only on physical health
and comfort, but also on emotional, spiritual and social well being. The team
approach recognizes that all of these aspects are connected. So the nurse
listens closely to the chaplain and they work with the social worker and the
volunteer because together they can get the most complete picture of a
patient's needs.
